Liability Waiver
ALBUQUERQUE, N.M. – The University of New Mexico beach volleyball team will be holding team tryouts on Monday, Sept. 10 at 2:30 p.m. for UNM students interested in joining the program.
Tryouts will be held at the volleyball courts located next to the Casas del Rio dorms on the main campus of UNM.
In order to try out, all interested students must fulfill a list of requirements before arriving for their tryout, including:
• Full-time enrollment as students (12 credit hours) at UNM for the Fall 2016 semester.
• A signed paper from a physician proving the student has undergone a physical within the last calendar year (performed anytime from Sept. 10, 2017 through Sept. 10, 2018).
• Proof of current health insurance (i.e., health insurance card or papers) from their health insurer.
• A liability waiver (linked above) signed by either the athlete or a parent/guardian if the athlete is under 18 years of age.
Interested students should send their interest in attending and their liability forms to Pauline Manser at pmanser@unm.edu no later than Thursday, Sept. 6th.
On the day of the tryout, students should arrive promptly for the tryout at 2:30 p.m., with all remaining required documents and wearing appropriate attire.
