2016 Lobo Softball Tryout Liability Waiver Form
ALBUQUERQUE, N.M. – The University of New Mexico softball team, under the direction of seventh-year head coach Erica Beach, will hold tryouts for possible walk-ons for its 2016-17 team on Wednesday, Sept. 7 at the Lobo Softball Field. Tryouts will be held at 3 p.m. The softball field is located off of University Blvd., just south of WisePies Arena.
The tryout is open to all female students enrolled full time for the fall 2016 semester. Each prospective participant must provide a copy of her medical insurance card, a copy of her fall 2016 class schedule at UNM, a copy of a doctor’s physical form within that last 12-18 months and a signed 2016 Lobo Softball Tryout Liability Waiver. If players are under 18, parents must sign waiver form.
If an athlete does not have medical insurance, they are not eligible for tryouts.
